Waiver Release of Liability:

I, the participant, acknowledge and understand that participating in, The Barney Davis 5K Run/ Walk involves inherent risks of injury, illness, or even death, as well as risk of danger to or loss of personal property. These risks may include but are not limited to, falls, contact with other participants, effects of weather, and road conditions. 

I willingly and voluntarily assume all such risks, both known and unknown, and accept full responsibility for participation. I hereby release, discharge, and hold harmless The Barney Davis 5K Run/Walk, its officers, directors, employees, volunteers, sponsors, and affiliated entities from any and all claims, liabilities, demands, actions, or causes of action to any loss, injury or damage I may sustain while participating in this event.

I certify that I am physically fit, have trained adequately for this event, and have not been advised otherwise by a medical professional. 

I grant permission for emergency medical treatment if necessary and acknowledge that any medical cost incurred will be my responsibility.
